The Quality Improvement and Innovation Partnership (QIIP)

Welcome to the QIIP Website. This website contains information about QIIP and its role in assisting Ontario's Family Health Teams as they move to a new model of primary health care. It contains information about activities that QIIP is organizing, including our collaborative for FHTs and CHCs, tools and resources to help FHTs with the work they are doing, and a forum for FHTs to learn about each other's achievements and activities and where people working in FHTs can connect with each other and exchange ideas and suggestions. Please visit the site map (above) to locate items of interest to your FHT or discipline.

We want this to be an interactive site, with resources being added on a regular basis, especially those produced by FHTs that will be of interest to their colleagues. We would encourage you to visit the site regularly and send us anything you think could be added.

New FHT and Nurse Practitioner-Led Clinic Workshop

The Ministry of Health and Long-Term Care has asked QIIP to host a workshop for the 20 new Family Health Teams, and 12 Nurse Practitioner-Led Clinics that were announced last fall. The two-day workshop will take place February 2nd and 3rd, 2010 and bring together representatives from established Family Health Teams to share best practice and lessons learned.

CAMH Survey of Ontario Primary Care Service Providers

QIIP was contacted recently by the Centre for Addiction and Mental Health (CAMH) regarding a project to develop resources for primary healthcare. This project is called Survey of Ontario Primary Care Service Providers Regarding Services and Resources Related to Mental Health and/or Addictions Concerns. You may wish to share this information with other team members within your FHT who are involved with mental health and substance abuse services and seek their feedback on service and resource needs.
