Learning Collaborative Three Report (March 2009 - May 2010)
Over a 14-month period, 51 primary care teams from across Ontario participated in Quality Improvement and Innovation Partnership’s Learning Collaborative 3. While keeping patient outcomes and experiences at the forefront, teams were able to demonstrate significant breakthroughs in the domains of chronic disease management, preventative care and office practice redesign.
