Chronic Disease Prevention and Management

There is an overlap between chronic disease prevention and management - and health promotion and illness prevention. Ontario has developed its own chronic disease prevention and management framework and model. Improvement Approaches

Improving Chronic Care, which also includes a guide to setting up a CDPM program and a prework package. They also have a tool to assess chronic illness care, which has been adapted slightly by the QIIP collaborative for the Ontario context
