Measuring outcomes is a key driver of improvement in Primary Care. FHTs need to be able to measure their current baseline functioning, the needs of their population, and the outcome of improvements. Using this information helps to drive further changes and innovations. FHTs also need to be able to evaluate the overall impact of preventive and health promotion interventions.

Health Canada is also working on a primary health care evaluation toolkit.
The World Health Organization has developed a Disability Assessement Schedule that provides information relevant and useful to researchers, clinicians, and administrators who are interested in learning about and using this instrument for assessing levels of functioning. |
