FHT Achievements

We want to use this page to showcase the achievements and accomplishments of Family Health Teams. This includes not only innovative programs or partnerships that FHTs have established, but also awards or external recognition that FHTs have achieved. We want to highlight achievements such as:

Family Practice of the Year Award

Congratulations to two QIIP Learning Collaborative Graduates - Marathon FHT and Regent Park CHC who won the 2009 Family Practice of the year award from the Ontario College of Family Physicians. Click here to learn more.

Dr. Neil Arya - Recipient of the Geeta Gupta Equity and Diversity Award (2009)

Awarded by the College of Family Physicians of Canada, Dr. Neil Arya of the Centre for Family Medicine was selected as recipient of this year's award as an outstanding family physician who has shown leadership and advanced awareness working in his practice and community to foster respect and understanding of minorities and under-serviced populations.

Centre for Family Medicine Doctor

Awarded by the Ontario College of Family Physicians, Dr. Linda Lee of the Centre for Family Medicine, was selected as this year's Family Physician for her work in patient care, dementia care, teaching and faculty development.

Dr. Joseph Lee - Recipient of Best Original Research Article Award (2009)

Awarded by Canadian Family Physician for an article entitled "Stress, burnout, and strategies for reducing them: What's the situation among Canadian family physicians?" February 2008

Doctor from Centre for Family Medicine wins Service Award for Geriatric Excellence

Please join us in congratulating Dr. Linda Lee of the Centre for Family Medicine in Kitchener for winning a Service Award for Geriatric Excellence. The award recognized Dr Lee's:
•exemplary performance and accomplishment in her area of responsibility
•leadership, innovation and creative thinking, decisive actions, cooperation and hard work in the provision of care or service that optimizes health, independence and quality of life of older adults
•and her utilization of best practice.
