This page allows FHTs to share with innovations in their practice, whether it’s a program, a form, a toolkit or a resource. If you have any materials that you think might be of interest to other FHTs, please forward them to us so we can include them on the site and share with your colleagues. This could include:
- HR and Governance documents - such as governance arrangements, interviewing guides, job descriptions, staff handbooks and policies. If these are posted on a website please send the link otherwise we will post the PDFs or Word files.
- Programs and program descriptions
- Tools and Resources to Support Clinical Management,
- Clinical Partnerships
