Quality Improvement
Optimizing our health care system requires change that leads to sustainable improvement in system processes and patient outcomes. The Model for Improvement, the Chronic Disease Prevention and Management Framework and adaptation of IHI’s Learning Model as outlined below, are proven methodological approaches at the foundation of QIIP’s work in primary healthcare in Ontario.
This is a very exciting time for primary healthcare. Since the creation of Quality Improvement and Innovation Partnership, QIIP has moved forward by coaching, training and supporting quality improvement processes in primary healthcare across Ontario. Specific focus has been on making sustainable improvement in the area of diabetes, cancer screening and access. This spring, we will expand to include focus on asthma, COPD and hypertension. Primary healthcare teams across Ontario continue to build improvement capability and capacity to make the necessary changes that lead to sustainable improvement in health care delivery for all patients.
