QIIP will help FHTs by preparing or disseminating tools and resources to assist them in their work and by facilitating an exchange of information and resources between FHTs. There are three ways we will do this, in addition to posting the videos of the presentations at LS1 of the QIIP collaborative.
A central tenet of QIIP has been the concept of FHTs learning from and supporting each other (FHTs helping FHTs). Your FHT may have produced materials that may be helpful to another FHT - such as a form, a patient handout, or a list of useful resources. Or you have a description of an innovative program you developed that may save colleagues in another FHT time and energy as they develop a similar initiative. If so, please forward it to Pierrette and we will be happy to post it on our innovation exchange page.

Many additional resources for health professionals and patients can be found on the Ontario MD site. This site requires registration, but FHT physicians can register all staff working in their FHT.
